A 76 year old female presented with weight loss and a strong family history of pancreatic cancer; 4 female family members had pancreatic cancer. A screening upper gastrointestinal endoscopic ultrasound (EUS) showed multiple pancreatic cysts (measuring ~12mm in size) present along the pancreatic duct. Also present was an irregular nodule in the body-tail junction that on repeat EUS had grown in size. EUS-guided FNA of this nodule was performed. Direct smears from 4 passes were made and a cell block prepared (Figures 1-3).
